North Texas Foster Care Consortium-Membership Application

This application is for membership in the North Texas Foster Care Consortium. Any organization involved in serving children in foster care in any of the counties in Region 3 is welcome to apply. (Please note that multiple individuals from the same organization can join the Consortium.) There is no membership fee.

4.Is your organization joining as a Regular (voting) or Affiliate (non-voting) member? 
Organizations that provide services to children in foster care (i.e. government agencies, businesses) are typically considered affiliate.

An organization is welcome to have more than one representative involved in the Consortium. Please have each representative complete a separate application.
